You are summoned and required to answer the petition which has been filed with the clerk of this court. Within 30 days after service of this summons, you must file your written answer with the clerk of the court at the following address:

74 South 100 East, Tooele, Utah 84074 and you must mail or deliver a copy to petitioner’s attorneys at the address listed above. If you fail to do so, judgment by default may be taken against you for the relief demanded in the petition.

This is an action for adoption of your child TAYLOR PATIENCE by DUSTIN R. DANGERFIELD and termination of your parental rights to your child TAYLOR PATIENCE.
